- If you have extended healthcare insurance that covers physiotherapy and would like us to try submitting your claims on your behalf, please download the Telus e-Claims Form, fill it out, and email to us or bring to your initial appointment. We will also require a copy of the card for your file so please scan to us, or bring it with you.
